Background

Ghor Province, located in central-western Afghanistan, remains one of the most underserved regions in terms of healthcare access. Geographic isolation, chronic poverty, climate-related disasters, and weak health infrastructure continue to limit service delivery, particularly in remote and mountainous districts.

In 2024, flash floods damaged or destroyed at least 27 health and nutrition facilities, further disrupting already fragile services. Poor road networks and seasonal access constraints severely restrict access to care, especially for women, newborns, and children.

Maternal and child health indicators remain critically high-risk. Limited availability of skilled female health workers and long distances to functioning facilities prevent timely access to antenatal, postnatal, and emergency obstetric care, increasing preventable maternal and neonatal mortality.

Malnutrition and communicable diseases such as acute watery diarrhoea, measles, and pneumonia are widespread, particularly among children under five and pregnant and lactating women, driven by food insecurity and inadequate WASH services.

Nationally, over 22.9 million people require humanitarian assistance, with 14.3 million in need of health services. Ongoing funding gaps threaten further reductions in health service coverage, disproportionately affecting hard-to-reach provinces like Ghor.

In response, the Afghanistan Green Crescent Organization (AGCO), in partnership with Amena Clinic and under AIMA oversight, implements an integrated fixed and mobile primary healthcare model aligned with national and UNICEF-supported service packages. Fixed clinics provide essential MNCH, outpatient care, nutrition, and preventive services, while mobile teams extend coverage to remote and underserved communities, ensuring continuity of care and strengthening referral linkages.

This combined approach has proven effective in increasing access, reducing delays in care, and improving early detection of complications. However, limited transportation and referral capacity remain critical gaps. A dedicated 4×4 ambulance is essential to ensure timely emergency referrals, expand outreach coverage, and strengthen the overall resilience of healthcare delivery in Ghor.

This intervention directly contributes to safeguarding the health and dignity of vulnerable populations, particularly mothers and children, while reinforcing a sustainable, community-trusted healthcare system aligned with national and international priorities.

About AGCO

The Afghanistan Green Crescent Organization (AGCO) is an officially registered Afghan non-governmental, non-political, and non-profit organization, established in 2017 as part of the International Green Crescent movement, which originated in Türkiye in 1920 and now operates in over 90 countries under the International Federation of Green Crescent (IGCF).

About Amena Mobile Clinic

Launched in November 2020, the Amena Mobile Clinic is a humanitarian health initiative designed to provide free, accessible, and quality healthcare to underserved populations in and around Kabul, as well as Kunar and Helmand provinces. Registered as a 501(c)(3) nonprofit organization in California, USA, Amena partners with Afghan institutions to deliver essential medical services to women, children, and the elderly who otherwise lack access to healthcare.

In collaboration with AGCO and the Afghanistan Islamic Medical Association (AIMA), Amena Mobile Clinic operates as a field-based mobile service, focusing on primary healthcare, maternal and child health (MNCH), preventive care, and psychosocial support. The Clinic also integrates public health awareness and addiction-prevention messaging in alignment with the Green Crescent mission.

Between 2020 and 2024, Amena Mobile Clinic delivered over 190,000 consultations, including thousands of maternal health visits. In February 2026 alone, the Clinic sustained high-impact outreach across multiple districts, attending to hundreds of patients daily and responding to emergency situations, including earthquake-affected communities. Social initiatives such as Eid Joy distributions have further strengthened community trust, ensuring that even during crises, vulnerable children and families receive care, support, and moments of hope.

  • Title: Midwife – Amena Fixed & Mobile Clinic
  • Location: Ghor Province (fixed clinic and mobile outreach to remote districts)
  • Reports to: Medical Doctor (Team Leader)
  • Supervises: None (provides technical support to female nurses, CHWs, and outreach staff)
  • Contract Type: Full-time
  • Duration: 6 Months, with possible extension based on performance and funding
  • Salary Grade: As per AGCO HR Policy

Purpose of Position

  • To deliver comprehensive, life-saving midwifery and reproductive health services through both fixed clinic and mobile outreach platforms in underserved and hard-to-reach areas of Ghor Province.
  • To contribute to reducing maternal and neonatal morbidity and mortality by ensuring access to quality ANC, PNC, safe delivery support, and timely referrals.
  • To strengthen community-based maternal and newborn care practices through outreach, awareness, and engagement with women and families.

Key Responsibilities

  1. Clinical Midwifery Services
  • Provide antenatal care (ANC), postnatal care (PNC), clean and safe delivery support, and essential newborn care in both fixed and mobile settings.
  • Conduct routine maternal health assessments including screening for anemia, hypertension, and pregnancy-related complications.
  • Identify high-risk pregnancies and obstetric complications, ensuring timely stabilization and referral.
  • Support Basic Emergency Obstetric and Newborn Care (BEmONC) services at clinic level where applicable.
  • Ensure strict adherence to infection prevention and control (IPC) standards in all clinical procedures.
  1. Maternal, Newborn & Child Health (MNCH) Integration
  • Support immunization follow-up and coordination with EPI teams during outreach activities.
  • Conduct nutrition screening (MUAC) for pregnant and lactating women and provide counseling on maternal nutrition.
  • Promote early initiation of breastfeeding and appropriate newborn care practices.
  1. Mobile Outreach Service Delivery
  • Actively participate in mobile health team deployments to remote, mountainous, and underserved communities.
  • Deliver on-site maternal health services, follow-up for pregnant women, and postnatal visits in areas with limited or no facility access.
  • Ensure continuity of care between mobile outreach services and fixed clinic referrals.
  1. Patient Care & Counseling
  • Provide culturally sensitive counseling on:
    • Family planning and birth spacing
    • Safe motherhood and danger signs in pregnancy
    • Birth preparedness and complication readiness
    • Breastfeeding and newborn care
  • Ensure respectful maternity care, maintaining dignity, confidentiality, and privacy at all times.
  1. Community Engagement & Health Promotion
  • Conduct community-based health education sessions for women, adolescent girls, and families.
  • Work with CHWs, community elders, and local leaders to improve health-seeking behavior and service utilization.
  • Promote awareness on maternal health, hygiene, and prevention of common diseases affecting mothers and children.
  1. Referral and Case Management
  • Identify emergency obstetric and neonatal cases and coordinate timely referral to higher-level facilities.
  • Support referral documentation and follow-up to ensure continuity of care.
  1. Teamwork & Coordination
  • Work closely with the Female MD Doctor, nurses, and other clinical staff to ensure integrated service delivery.
  • Participate in case discussions, team meetings, and clinical reviews.
  • Support the Team Leader in ensuring quality standards across services.

 

  1. Reporting & Health Information Management (HMIS)
  • Maintain accurate and complete records of:
    • ANC and PNC visits
    • Deliveries conducted/supported
    • Family planning services
    • Referrals and complications
  • Contribute to monthly HMIS reporting and data quality assurance.
  1. Compliance & Accountability
  • Ensure all services are delivered in line with MoPH guidelines, WHO standards, donor requirements, and AGCO policies.
  • Uphold ethical medical practice and accountability to beneficiaries, especially vulnerable women and children.

Required Qualifications, Skills, and Competencies

Education

  • Diploma or Bachelor’s degree in Midwifery from a recognized institution
  • Valid registration/license from the Ministry of Public Health (MoPH)

Experience

  • Minimum 3 years of relevant midwifery experience, preferably in BPHS/EPHS or humanitarian settings
  • Proven experience in maternal, newborn, and child health (MNCH)
  • Experience in mobile health services or rural/remote service delivery is strongly preferred

Skills

  • Strong clinical competency in midwifery and reproductive health
  • Ability to manage cases in resource-limited and high-risk environments
  • Excellent communication and counseling skills with women and families
  • Fluency in Dari and/or Pashto; basic English preferred
  • Basic computer skills (HMIS, MS Office) are an advantage

Working Conditions

  • Duty Station: Ghor Province, with frequent travel to remote and hard-to-reach districts
  • Schedule: Full-time (Saturday–Thursday), with flexibility for outreach missions
  • Environment: Field-based, including mobile clinic operations in challenging terrain and weather conditions.
