Background

Context

Ghor Province, located in central-western Afghanistan, remains one of the most underserved regions in terms of healthcare access. Geographic isolation, chronic poverty, climate-related disasters, and weak health infrastructure continue to limit service delivery, particularly in remote and mountainous districts.

In 2024, flash floods damaged or destroyed at least 27 health and nutrition facilities, further disrupting already fragile services. Poor road networks and seasonal access constraints severely restrict access to care, especially for women, newborns, and children.

Maternal and child health indicators remain critically high-risk. Limited availability of skilled female health workers and long distances to functioning facilities prevent timely access to antenatal, postnatal, and emergency obstetric care, increasing preventable maternal and neonatal mortality.

Malnutrition and communicable diseases such as acute watery diarrhoea, measles, and pneumonia are widespread, particularly among children under five and pregnant and lactating women, driven by food insecurity and inadequate WASH services.

Nationally, over 22.9 million people require humanitarian assistance, with 14.3 million in need of health services. Ongoing funding gaps threaten further reductions in health service coverage, disproportionately affecting hard-to-reach provinces like Ghor.

In response, the Afghanistan Green Crescent Organization (AGCO), in partnership with Amena Clinic and under AIMA oversight, implements an integrated fixed and mobile primary healthcare model aligned with national and UNICEF-supported service packages. Fixed clinics provide essential MNCH, outpatient care, nutrition, and preventive services, while mobile teams extend coverage to remote and underserved communities, ensuring continuity of care and strengthening referral linkages.

This combined approach has proven effective in increasing access, reducing delays in care, and improving early detection of complications. However, limited transportation and referral capacity remain critical gaps. A dedicated 4×4 ambulance is essential to ensure timely emergency referrals, expand outreach coverage, and strengthen the overall resilience of healthcare delivery in Ghor.

This intervention directly contributes to safeguarding the health and dignity of vulnerable populations, particularly mothers and children, while reinforcing a sustainable, community-trusted healthcare system aligned with national and international priorities.

About AGCO

The Afghanistan Green Crescent Organization (AGCO) is an officially registered Afghan non-governmental, non-political, and non-profit organization, established in 2017 as part of the International Green Crescent movement, which originated in Türkiye in 1920 and now operates in over 90 countries under the International Federation of Green Crescent (IGCF).

AGCO’s mission is to protect individuals, families, and communities from harmful practices and addictions—including tobacco, drugs, alcohol, gambling, and technology misuse—while promoting health, dignity, and resilience. In Afghanistan, AGCO has expanded its mandate to include primary healthcare services, mobile clinics, psychosocial support, disaster and emergency response, and community resilience programs.

Operating across Kabul, Kunar, and Helmand provinces, AGCO has reached tens of thousands of vulnerable families. Its emergency response capacity has been demonstrated through rapid interventions following earthquakes, natural disasters, and other crises. The organization also implements social support initiatives, including distribution of Eid toys and community outreach events, reinforcing social cohesion and providing relief and hope to affected populations. Through these integrated programs, AGCO has established itself as a trusted partner for international donors, government institutions, and local communities.

  1. b) About Amena Mobile Clinic
    Launched in November 2020, the Amena Mobile Clinic is a humanitarian health initiative designed to provide free, accessible, and quality healthcare to underserved populations in and around Kabul, as well as Kunar and Helmand provinces. Registered as a 501(c)(3) nonprofit organization in California, USA, Amena partners with Afghan institutions to deliver essential medical services to women, children, and the elderly who otherwise lack access to healthcare.

In collaboration with AGCO and the Afghanistan Islamic Medical Association (AIMA), Amena Mobile Clinic operates as a field-based mobile service, focusing on primary healthcare, maternal and child health (MNCH), preventive care, and psychosocial support. The Clinic also integrates public health awareness and addiction-prevention messaging in alignment with the Green Crescent mission.

Between 2020 and 2024, Amena Mobile Clinic delivered over 190,000 consultations, including thousands of maternal health visits. In February 2026 alone, the Clinic sustained high-impact outreach across multiple districts, attending to hundreds of patients daily and responding to emergency situations, including earthquake-affected communities. Social initiatives such as Eid Joy distributions have further strengthened community trust, ensuring that even during crises, vulnerable children and families receive care, support, and moments of hope.

Position Details

  • Title: Medical Specialist (Doctor) and Team Leader of Amena Mobile Clinic

  • Supervises: Nurses, Midwives, and other clinical staff
  • Contract Type: Full-time
  • Duration: 6 months  with possible extension based on performance and funding
  • Salary Grade: As per AGCO HR Policy

Purpose of Position

  • To provide quality medical diagnosis, treatment, and preventive healthcare to patients of the Amena Mobile Clinic.
  • To ensure adherence to national health protocols and international medical standards in the delivery of healthcare services.

Key Responsibilities and Duties

  1. Clinical Service Delivery:
    Provide high-quality, patient-centered clinical care to men, women, and children in both fixed and mobile clinic settings, in accordance with MoPH national treatment protocols (including IMCI, TB-DOTs, Malaria, and NCD management). Conduct consultations, physical examinations, diagnosis, and treatment, ensuring rational use of medicines and adherence to infection prevention and control (IPC) standards.
  2. Maternal, Child Health & Preventive Services:
    Deliver essential maternal and reproductive health services, including antenatal and postnatal care, family planning counseling, and referrals for complicated cases. Ensure routine immunization services are provided in coordination with EPI teams, and promote preventive healthcare through community awareness on hygiene, nutrition, and disease prevention.
  3. Mobile Health Team Leadership:
    Lead and coordinate mobile health team operations in hard-to-reach and underserved areas of Ghor province. Plan outreach schedules, ensure timely deployment of services, supervise team members (nurses, midwives, vaccinators, CHWs), and ensure quality and consistency of services delivered in remote communities.
  4. Community Engagement & Outreach:
    Strengthen community-based health systems by supervising and mentoring Community Health Workers (CHWs) and supporting health posts through regular field visits (at least monthly). Enhance community trust, health-seeking behavior, and referral linkages between communities and health facilities.
  5. Referral and Case Management:
    Identify complicated, severe, or high-risk cases and ensure timely and appropriate referral to higher-level facilities, with proper documentation and follow-up. Maintain coordination with referral centers to ensure continuity of care.
  6. Health Information Management & Reporting (HMIS):
    Ensure accurate patient registration, proper documentation of clinical services, and timely submission of monthly and quarterly HMIS reports. Maintain data quality, confidentiality, and use data for decision-making and service improvement.
  7. Clinic & Team Management:
    Oversee day-to-day operations of fixed and mobile clinics, including staff supervision, task allocation, and performance monitoring. Ensure availability and proper management of medical supplies, equipment, and pharmaceuticals. Maintain cleanliness, safety, and functionality of clinic environments.
  8. Emergency Preparedness and Response:
    Provide first aid and manage emergency and life-threatening conditions in both static and mobile settings. Support rapid response during disease outbreaks, natural disasters, or humanitarian emergencies in coordination with AGCO and local health authorities.
  9. Capacity Building & Coordination:
    Participate in and facilitate regular team meetings, case discussions, and training sessions. Build the capacity of junior staff and CHWs through on-the-job training, mentoring, and supportive supervision. Coordinate closely with MoPH, NGOs, and other stakeholders operating in Ghor province.
  10. Compliance & Accountability:
    Ensure all activities are implemented in line with AGCO policies, donor requirements, and MoPH guidelines. Promote accountability, transparency, and ethical medical practice, with a strong focus on serving vulnerable and underserved populations.

Job Requirements

  1. Education:
    Medical Doctor (MD) degree from a recognized university, with valid MoPH license and successful completion of the MoPH Exit Exam.
  2. Experience:
    Minimum 2–3 years of relevant clinical experience, preferably within BPHS/EPHS facilities or humanitarian health projects. Prior experience in mobile health teams or remote service delivery is highly desirable.
  3. Languages:
    Fluency in Dari and/or Pashto is required; working knowledge of English is preferred.
  4. Technical Skills:
    Strong clinical and diagnostic skills, familiarity with national treatment guidelines, and competency in HMIS reporting. Proficiency in MS Office applications.
  5. Core Competencies:
    Excellent leadership, communication, and interpersonal skills. Ability to work under pressure in challenging and resource-limited environments. Willingness to travel frequently to remote areas. Strong commitment to humanitarian principles and serving vulnerable populations.
